Two individual's, one teaching staff and one kitchen staff were hired and working in the program without having been fingerprinted or the criminal history process initiated. .
One staff member has an expired PDL and did not resubmit in time to maintain a current PDL
The minimum number of hours were not obtained to maintain the professional development ladder certificate for one staff member
Refrigerator in infant room being used to store a child's bottle did not have a thermometer.
3 staff do not have current CPR or first aid training and have been employed over 3 months.
During the visit, personnel informed licensing that the central detection system was not working or active at this time. Licensing tested the carbon monoxide detector while on site and it was unresponsive.
Fire drill had not been conducted or conducted since April of 2023.
Tornado drill had not been conducted or documented since April of 2023.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Licensing Requirements were reviewed. Appropriate sleep environment requirements posted in classroom. Classroom teacher will retake Safe Sleep training.

340:110-3-296(b)(2)(B)(ii) - When an infant falls asleep in equipment other than appropriate rest equipment, the infant is immediately moved to appropriate rest equipment.

Additional Non-Compliance Found During Investigation: Equipment: (SERIOUS) Infant was observed sitting in a swing for a period longer than 20 minutes.
Plan to Correct
Licensing requirements reviewed. Licensing Training scheduled with OHS on March 14, 2025. Education manager will meet with teaching staff weekly to ensure licensing requirements are being met and are understood.
Regulation Description
340:110-3-289(j)(3) - do not remain for more than 20 minute increments in equipment restricting freedom of movement, such as rest equipment, swings, high chairs, or stationary activity centers, except while eating in high chairs or similar stationary equipment. Children only sleep in appropriate rest equipment, per OAC 340:110-3-296.

Changing table moved to accommodate teachers being in reach the counter and sink. Supervision training scheduled with DHS March 14, 2025.

Supervision training with OHS scheduled 3/14/25. Disciplinary Actions have been conducted - termination of one staff member and written disciplinary action for other staff member involved.

340:110-3-287(c) - Non-job related. do not participate in personal activities that may interfere with supervision, such as visitors, phone calls, or electronic device use.

Rest time: (SERIOUS) Infants are wrapped in blankets in their cribs and pillows are in the cribs with infants while asleep.
Plan to Correct
Blankets and pillows have been removed from all cribs. Appropriate sleep environment requirements have been posted in the classroom. Director will have daily contact with each classroom to ensure requirements are being met.

340:110-3-296(b)(2)(B)(iii) - Soft products, such as quilts, comforters, sheepskins, pillows, stuffed toys, and bumper pads are prohibited inside and on the side of infant rest equipment.
