340:110-3-289(j)(3) - do not remain for more than 20 minute increments in equipment restricting freedom of movement, such as rest equipment, swings, high chairs, or stationary activity centers, except while eating in high chairs or similar stationary equipment. Children only sleep in appropriate rest equipment, per OAC 340:110-3-296.

Supervision training with OHS scheduled 3/14/25. Disciplinary Actions have been conducted - termination of one staff member and written disciplinary action for other staff member involved.

Blankets and pillows have been removed from all cribs. Appropriate sleep environment requirements have been posted in the classroom. Director will have daily contact with each classroom to ensure requirements are being met.

Staff were given a verbal warning about tobacco in the facility. Tobacco products are not allowed in the facility and must be kept in personal vehicles. Staff will complete two online trainings on the Take Down Tobacco Website - "Take Down Tobacco 101" and "The Rise of Vaping".
