One teaching personnel was hired with a current OPDL certificate and it is now expired, therefore, they did not maintain a current PDL.
Worker observed the impact of the outdoor play equipment, with a fall height of 4 ft or higher, is not being maintained at a depth of at least six inches.
Worker observed one child not signed in on this date.
When worker arrived, the teacher in the infant and 1-year-old room stepped out of the room leaving the ratio 4 children to 0 personnel. Another classroom had a ratio of 13 children to 1 personnel with one 2 year old.

Licensing observed all 26 enrolled children signed in on this date; however, only 22 children were actually present at the center on this date.
One teaching personnel employed at the program for 14 days and having sole responsibility of a group of children, does not have orientation within one week of employment date

In classroom of 3 year olds, the teacher left the classroom to take one child to the restroom leaving the children that were napping with no staff in the room.

Corrected on day worker was here. Had a staff meeting on 6/13/25 with current staff on maintaining supervision at all times no matter what. If staff need to leave the room, they must notify the director to make sure supervision and ratios are being maintained at all times.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
