  • *Serious* Licensing observed a personnel that arrived to the program at 11am to work. Background investigation was not submitted for personnel prior to personnel working. Licensing called OBI. OBI informed Licensing that personnel had fingerprints received on 1/14/25 but not BIDS request completed.
  • Licensing observed the outside playground not in working condition. Licensing observed a wood table with metal legs upside down, large plastic toys laying on sides, basketball goal backboard broken and broken pieces laying on the ground and did not have a net, a board laying in the middle of the playground, and a 2x4 pieces of cut wood in play area.

  • The outdoor play area and other areas that are accessible to children have a drainage ditch that is hazards. The ditch in the play area has metal pieces and other hazards.
  • Licensing observed a child latch on the refrigerator door. Health inspection completed on 11/14/24 stated do not use, door won't stay shut.
