Non Compliance Observed Licensing observed a personnel, caring for an infant(s), Safe Sleep training obtained was not approved formal professional development. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Licensing had personnel that was not caring for infant, with correct training to switch with the personnel without the correct training. Owner will get with director and ensure that any new personnel that has Safe Sleep prior to hire, that the personnel's training is approved by verifying on okregistry. Owner will have personnel in Safe Sleep by 11/21/25. In the future will maintain requirements. Regulation Description Safe sleep. Non Compliance Observed Licensing observed a mixed ratio classroom of 9:2 with 1 infant playing indoors on an inflatable bounce house. Numerous, Repeated, and/or Serious Non Compliance Yes Numerous: No Repeated: No Serious Non Compliance: Yes Plan to Correct The children were immediately removed off bounce house. Licensing went over high risk activities with the owner and showed in requirements. Owner stated was not aware that the bounce house was considered high risk due to in not being fully enclosed. Owner stated that it is hardly used and will remove it from the property and not use it again. In the future will maintain requirement. Regulation Description Higher risk activities. Higher risk activities, such as swimming, shoreline activities, archery, ice or roller skating, gymnastics, enclosed inflatable bouncing equipment, or karate increase the risk for severe injuries.
