Two children (over 12 months) in 2 & older classroom had cups in rest equipment. Two other children over 12 months in the other classrooms had bottles in rest equipment.
In the Infant & older class, the staff member left the room on four separate occasions while Licensing was on site.
Three children did not have a Compliance File or Insurance Notification form completed. Seven children's Compliance File & Insurance Notification forms were not signed or dated.
