Non Compliance Observed Three children did not have a Compliance File or Insurance Notification form completed. Seven children's Compliance File & Insurance Notification forms were not signed or dated. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Program will have the parent/guardian complete the children's Compliance File & Insurance Notification forms. Program is revising the Program's Child Packet. Program will make sure that all required forms are completed before children begin care. Regulation Description Parents complete DHS forms every 12 months regarding insurance liability and the compliance file. Forms are maintained at the facility. ¢ 6 Plan to Correct Date 2025-04-21
Ratios
Non Compliance Observed In the Infant & older class, the staff member left the room on four separate occasions while Licensing was on site. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct This was corrected once staff returned to classroom. Ratios will be reviewed with all staff at the next staff meeting. Regulation Description Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times. Plan to Correct Date 2025-04-16
Eating and drinking location
Non Compliance Observed Two children (over 12 months) in 2 & older classroom had cups in rest equipment. Two other children over 12 months in the other classrooms had bottles in rest equipment. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct This was corrected when staff removed the bottles/cups from rest equipment. This will be reviewed & addressed with all staff members at the next staff meeting.
