- Two staff employed past 90 days have not completed ELCCT training or equivalent.
- Infant teacher walked out of the classroom and into the front entry area for a short period, leaving the group unattended and out of ratio (2 infants and a 1-yr-old); a teacher with the 5 and older group left the classroom for a few minutes, leaving the group with one teacher and 26 children; in addition, three new staff could not be considered in staff-child ratios due to criminal history reviews (background checks) not having been submitted. At 12:35 PM, only one teacher was present with the 5 and older group of 26 children; children were resting but the second teacher left for break and did not remain in the building.
- Two children, ages 1 and 2, were present in a classroom with no diapering area or handwashing sink; the children are not in the process of potty training and are still in diapers; this is the children's assigned classroom.
- Criminal history review requests (background investigations) have not been submitted for three staff present today; hire dates for the three staff are 6/28/23, 7/18/23 and 7/19/23.
- The teacher for the infant/1-yr-old classroom left the room to go to the lobby area for a short period, two infants and a 1-yr-old were left unsupervised.
