Licensing observed in the hallway were cubbies are located a missing ceiling tile with hanging light fixture and insulation exposed. Another room not being used at the time with ceiling tile missing with lighting and insulation exposed.
Licensing observed 3 personnel with expired PDL's. HH PDL expired on 3/2/24 only has 2 hours of training and EP PDL expired on 11/18/23 with no completed training. No application has been submitted for either personnel.

Licensing observed program's last fire inspection was conducted on 1/27/22.
(Serious) Licensing observed an infant asleep in a crib with a pacifier. Infant was asleep on stomach and pacifier was not in the infants mouth. Licensing picked up pacifier and pacifier was attached to the infant's clothing.
(Serious) Licensing observed one personnel working without a submitted back ground to OBI. Personnel has been employed since 2/15/24.
Licensing observed in the kitchen a "Food Anaphylaxis Action Plan" that had been submitted by parent from the doctor's office. Licensing asked director to view medication permission form for child and director did not have one on file.
Licensing observed in the hallway were cubbies are located a missing ceiling tile with hanging light fixture and insulation exposed. Another room not being used at the time ceiling tile was missing with lighting and insulation exposed.
Licensing observed 3 personnel with expired PDL's. Licensing checked with CECPD regarding personnel PDL's. HH PDL expired on 3/2/24, no application has been submitted for renewal and currently has 2 hours of training. MH PDL expired on 10/21/23, no application has been submitted for renewal, transcript was submitted and CECPD received it in Dec of 2023. EP PDL expired 11/18/23, no application has been submitted for renewal, no training has been completed or training sent for review since 2022.
Licensing observed that the program currently does not have a Qualified Master Teacher.
(Serious) When licensing entered the first room in the program personnel left to get director. The second room licensing entered was the room director stated she was currently overseeing. Director continued with licensing, while licensing walked to 3 other classrooms with children left unattended in the second room.
