  • When Licensing arrived, there was one teacher with 10 children in the front classroom; children were ages one and older (one child at age 1); two other staff were sitting at a table in the main large area of the center.
  • An infant was sound asleep in a bouncy seat; the teacher in the classroom was not involved in another task and was sitting near this infant and two other awake infants, also in bouncy seats.
  • A bottle was propped on a blanket on infant's chest area while the child was consuming the bottle.
  • An infant, age four months, was not being held while consuming a bottle.
  • A four-yr-old child was restrained in a high chair for punishment; the high chair was placed against a wall and turned backwards with the child facing the wall; the child was buckled into the high chair.
  • Electrical outlets in the new classroom being used for school-age children do not have outlet covers in place; staff present stated children ages five and older use this classroom.
  • Three staff employed past three months have not obtained CPR/1st aid training; three other staff do not have current CPR/aid training (specific dates are noted in discussion)
  • The teacher in the ages three and older classroom was heard yelling at children out of anger during the monitoring visit.
  • Three personnel employed past 12 months have not obtained an OPDL certificate; three staff employed past 12 months have expired OPDL certificates - expiration dates are 5/14/21, 7/05/24 and 7/25/24
  • Four staff employed past 90 days have not completed ELCCT training or equivalent training.

  • The classroom with children ages five and older was very chaotic; one child was hitting several other children which had to be brought to the director's attention by other children and by licensing; other children were rough-housing; other classrooms and children also required the director's attention during this time (director was the teacher for the classroom).
  • A staff person who is not 21 years of age has been transporting children to and from school; driver is age 20.
  • Director is not listing staff on OPDR. Staff are self-reporting to the OPDR.
  • Three staff employed past 12 months have not obtained a OPDL certificate; one staff has not maintained a current OPDL certificate, expired 5/14/21.
