Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
When licensing arrived an infant was asleep in a bouncy seat and the infant while in the bouncy seat was placed in a play yard.
A bib was around an infant neck while sleeping.
One personnel that was rehired in August of 2023 and did not resubmit a criminal history review request form to receive a new completed date. The program submitted a new request in 3-3-22 and the status was incomplete closed.
Director's ODC expired 12-9-23
Master teacher assigned CPR/FA expired in May of 2023
Two personnel did not complete CPR/FA within the 90 days or hire. One personnel started employment in October of 2023 and the other employee started employment in June of 2023
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
