  • When licensing arrived an infant was asleep in a bouncy seat and the infant while in the bouncy seat was placed in a play yard.
  • A bib was around an infant neck while sleeping.
  • One personnel that was rehired in August of 2023 and did not resubmit a criminal history review request form to receive a new completed date. The program submitted a new request in 3-3-22 and the status was incomplete closed.
  • Director's ODC expired 12-9-23
  • Master teacher assigned CPR/FA expired in May of 2023
  • Two personnel did not complete CPR/FA within the 90 days or hire. One personnel started employment in October of 2023 and the other employee started employment in June of 2023
