The direct care organization is not being maintained with current information.
Current CPR and First Aid certification could not be verified for one teacher employed longer than 12 months.
Tornado drills were not documented for the last month and relocation/evacuation drills were not documented for the last year.
Physical environment checklist has not been completed for 13 months.
The one year old and older classroom had 11 children to one staff with 9 one year olds. Two personnel were present but one personnel was resting and could not be counted in the ratio. This ratio is required to revert back to the straight ratio for one year olds since the total number one year olds was more than 2 per personnel so the ratio was required be reverted back to the straight ratio for one year olds since that was the youngest age in the classroom. The straight ratio for one year olds is one personnel to 6 one year olds.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Plan to Correct
Director will work with owner to get the slide removed and
get another one. If this is not possible right away, director
will get the slide removed from the playground so the kids
don't get hurt. If this is not immediately possible, director
will block access to the slide with cardboard or
posterboard, and assign an extra teacher to keep children
off the slide when children are on the playground.

Personnel (Responsibilities) - Staff did not ensure the health and safety of two children who were not picked up from school.
Plan to Correct
Give Rockwood school a list of the children that ride Oak Grove Day School Bus. I will continue working with staff at Rockwood school to make sure they know what children ride the bus. Make sure the staff do not have to pick children up in their cars. If children not enroll in day care at Oak Grove Daycare, I will ask the school to call their parents we cannot pick up any child that are not enroll in daycare for their safety and health.
