Teaching personnel are unable to identify or recognize children assigned to their group because children are routinely moved between classroom. Licensing inquired about the ages of the children in their care. Staff were unable to provide the ages of the children.
Two teaching staff that have been employed for 12 months or longer do not have current OPDL.
During today's visit staff and children were observed on the playground. The exterior door was locked and staff did not have a means to enter the building without an individual inside the building opening the door. When a child needed to use the restroom, the toileting facility was not easily accessible.

The program is limiting children's access to play equipment in two classrooms.
Licensing arrived at the program at 3:01pm upon arrival licensing rang the door bell and knocked on the door multiple times. Licensing called the center's main phone line as well as attempting to contact the program's owner via telephone. Staff acknowledged licensing's arrival through the window of a classroom, but did not make any effort to open the locked door for licensing. Licensing was finally let into the program at 3:28pm by the assistant director.

One classroom was over ratio today - The room had 2 infants, 6 one's year old's, 2 two's year old and 1 thirteen year old. The mixed-group only allows this room to have no more than two under two years of age per teaching personnel. Licensing observed a 1 five year old was in the hallway outside of the infant classroom. Several staff members were asked about the child and where they belonged before the child was returned to their group.
Licensing observed one five year old child in the hallway by the infant room unsupervised with no teacher in sight or near by. Several staff members were asked about the child and no one could identify which classroom the child belonged to.
The program's Registry profile is not being maintained as at least four personnel listed on no longer work for the program.
