This letter is to formally document the corrective actions taken in response to an incident involving a staff member who struck a child with a shoe while in care at Kreative Kidz Learning Academy. Immediately upon learning of the incident, the staff member involved was terminated from employment. The safety and well-being of every child in our care is our highest priority, and we maintain a zero-tolerance policy for any form of inappropriate discipline, abuse, or neglect. An internal review was conducted promptly, and all required reporting procedures were followed in accordance with Oklahoma DHS Child Care Licensing requirements. We fully understand the seriousness of this matter and have taken the following corrective measures to prevent any future occurrence: 1. All staff have completed additional training on child abuse and neglect recognition, prevention, and mandatory reporting requirements. 2. Refresher training was conducted on active supervision practices, appropriate discipline policies, and maintaining professional conduct at all times. 3. Staff were reminded that if they feel overwhelmed or unable to perform their duties appropriately, they are required to immediately notify administration and may be relieved from duty for the remainder of the day. 4. Additional training was provided regarding proper use of classroom materials and equipment in accordance with manufacturer guidelines and safety standards. 5. Supervision monitoring procedures have been reinforced to ensure consistent accountability and oversight. Kreative Kidz Learning Academy remains fully committed to providing a safe, nurturing, and developmentally appropriate environment for every child in our care. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Regulation Description

340:110-3-280(b)(1) - Abuse and neglect. Any person who has reason to believe a child was abused or neglected, per Section 1-1-105 of Title 10A of the Oklahoma Statutes (10A O.S. § 1-1-105) is required to report the matter promptly to the OKDHS Child Abuse and Neglect Hotline at 1-800-522-3511, per O.S. § 1-2-101. Failure to report is a misdemeanor offense and upon conviction is punishable by law. Failure to report with prolonged knowledge, six months or more, of ongoing abuse or neglect is a felony offense.

Plan to Correct

We have fixed the toilet, and we do plan to update it. It was back to working by the time that worker come back on 12/2/2025. We will keep it working we had plumbers and other means working on it to get it fixed when the time of the complaint, we were trying to get it fixed right. If this happens again, we will let licensing know and let them know we are working on it.
