Non Compliance Observed Licensing requested personnel information sheets for 5 staff during the visit. Staff in charge reported that the staff have been employed over two weeks. Staff could not find the forms during the visit. Non Compliance Observed Licensing observed the toilet to be removed from the bathroom in the middle classroom and sitting on the shelf. Staff reported they were having it replaced due to it not working. Non Compliance Observed Licensing observed the ground to be tore up in the outdoor play area, creating large holes, and exposed piping. Licensing observed a shovel next to the holes.
