- When workers arrived staff answered the front door with 2 children, 5 children were left alone in their classroom while the staff opened the door. Workers also observed a 2 year old child completely alone coming from the kitchen of the facility. Workers walked thru looking to see if staff was with him but he was completely alone.
- Personnel left in charge of the program is 20 years old.
- Workers arrived at 9:56am to conduct a full monitoring visit, staff did not allow licensing in until 10:20am. Workers had contacted director and she said she was on a field trip at the gathering place with children, she said the children at the center did not have permission slips and that Kiki was at the center. But when owner/director showed up she had no children and said Kiki was at therapy and she had forgotten.
