Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
No recent Quarterly inspection was conducted or documented for transportation vehicle. The program was unable to provide CCL with the Quarterly Inspection checklist.
When workers arrived staff answered the front door with 2 children, 5 children were left alone in their classroom while the staff opened the door. Workers also observed a 2 year old child completely alone coming from the kitchen of the facility. Workers walked thru looking to see if staff was with him but he was completely alone.
Personnel left in charge of the program is 20 years old.
Workers arrived at 9:56am to conduct a full monitoring visit, staff did not allow licensing in until 10:20am. Workers had contacted director and she said she was on a field trip at the gathering place with children, she said the children at the center did not have permission slips and that Kiki was at the center. But when owner/director showed up she had no children and said Kiki was at therapy and she had forgotten.
Director did not maintain transportation attendance or have emergency contact information with her while transporting children for the program.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
