Two directors listed, 6 persons need to be associated after working 12 months, and 19 persons have been associated to the DCO account that no longer work for the program since last visit.
One support staff has not submitted a criminal history review request after being fingerprinted; one teacher needs to resubmit a CHRR due to clerical error.
Diaper changing procedures were not followed by one teacher observed.
Three teachers employed longer than 90 days do not have evidence of having obtained Pediatric CPR/FA certification.
SERIOUS: Three teachers passed Licensing in the foyer and entered 3 different classrooms where they were needed to meet staff/child ratios (all 2's, 3 and older, and all 1's classrooms); One teacher used in ratio in the 3 and older classroom exited the classroom into the hallway and re-entered the classroom prior to the second teacher arriving into the room.
Mother's Day Out program (First Learning Center) and the licensed Part-Day program (First Learning Center Plus) are operating in the same licensed classroom space.
One teacher in the 6:2 all infant classroom was breastfeeding her child and being used to meet staff/child ratios; a teacher in the all 2's classroom walked out of the classroom into the hallway to place a portable crib in the hallway.
One infant was observed sleeping in a non-moving swing & another infant was observed sleeping in a bouncer in the 6:2 all infant classroom.
1 teacher does not have evidence of having been fingerprinted. (CHRR submitted to OBI on 3/11/24)
2 teachers have invalid preliminary results, 2 teachers have incomplete results.
Written parent permission not on file for an infant girl swaddled in a pink, velco swaddler.
Light level in one classroom was not light enough to see children's faces.
Items to be posted in prominent location at main entrance are posted on the south hallway wall where north hallway parents are not able to clearly view them.
