Posted lesson plans in the school age classroom were dated 11/3/25-11/7/25 and 10/6/25-10/10/25; lesson plans posted in the 2's classroom are dated for 10/24/25.
Equipment inventory last conducted 11/1/24.
Last quarterly inspection for both vans was 5/15/25.
Licensing cannot verify director and master teacher attendance due to the records being sent to payroll.
Driver attendance, CPR/FA certification, and Orientation form are being maintained at a separate center location owned by same owner.
One staff member who was previously a teacher in 2022 and turned substitute has an expired PDL certificate.
Lesson plans were outdated for the 3 classrooms in use today (infant room - February; school age room- January; 2 & older - July 2024, January, & March
7 screws are protruding from small outdoor playground fence on east side (6 at 1 foot above ground level & 1 at second brace bar)
The child to staff ratio was 7:1 with one infant and one toddler, remaining children were two years. The classroom is out of ratio due to the child to staff ratio of 6:1.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

1) Children that are known biters will be placed by the teacher and given an age appropriate activity during diaper changes. 2) Mirror was mounted to wall to give teacher better visibility of children during diaper changes.

Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
