Licensing observed one employee, employed longer than 90 days that has not completed ELCCT.
Licensing observed four children that did not have a compliance file notification.
Licensing observed one teacher employed longer than three months, without a current CPR/1st Aid certificate.
Licensing observed one child record that did not have a Child Information Form
Licensing was unable to view an updated Physical Environment checklist and the last one expired 7/11/24.

Plan to Correct
Have had coaching with staff through Ivy Resource and Referral as well as QRIS coach, will continue to monitor staff and coach on appropriate tone and language.

340:110-3-288(c)(2)(C) - yelling at a child out of anger or using harsh or profane language;
Complaint:
Personnel: Program Director has not been at the Program in over two weeks.
Plan to Correct
New Director installed on 7/21/25. We will ensure a new director or acting director is put in place in the event the current director will be absent for what may be a prolonged time in the future.
Regulation Description
340:110-3-284.1(b)(2)(C) - present at the facility during the hours of operation at least 50 percent of the operating hours or 30 hours per week, whichever is less; and
Complaint:
Personnel: Staff allowed a parent to care for a child (not related) while it was in care.
Plan to Correct
Staff will not allow any person or parent to pick up or care for another child in the program. Director or staff in charge will ensure all employees coming into the program are aware of the requirements.

Health Protection & Disease Control: Program Staff are vaping around the children.
Plan to Correct
Per the Notice to Comply dated 6/07/23:
All current employees are aware that vaping is not allowed in or outside the facility. Employees will sign a smoke-free, vape-free, drug-free policy when hired that will be placed in their employee file. Discipline action will be taken if the policy is violated and could result in termination.
