2 teaching personnel have expired PDL's.
3 staff employed for 12 months do not have PDL's associated with the programs registry.
A person listed on the programs registry as a current employee does not meet qualifications to be employed by the program.
Licensing observed 13 children in one van; the van has 3 bench seats behind the driver/passenger seats where children were present; the van has space for nine passengers, 3 passengers per row. 5 children were observed in the first row and the other 8 children were in the second and third rows.
Five children, ages 5 through 7, were transported with no child passenger restraints in place; no booster seats were in place for the children and the children were not using seat belts. Eight children older than age 7 were transported in the second and third rows where only six seat belts were present.
3 teaching personnel employed for 90 days do not have documentation of the completion of entry level training.
2 teaching personnel employed 90 days have not obtained cpr/first aid certification.
1 teaching personnel employed has an expired cpr/first aid certifications.

The program fire extinguisher was last inspected and tagged in August of 2023.
Two individuals present and working with children have criminal history restrictions; notice of restriction was previously provided to owner/director for both individuals; for one of the individuals, a waiver request was denied.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Transportation - Children are being transported without any child passenger restraints and/or booster seats in place.
Plan to Correct
A plan of correction was received 9/30/24 from the program's owner/director Felicia Logan - The booster seats will be moved from under the seats and be placed in the seats for the children to use. The program will ensure booster seats are used for children under 8 years of age and seat belts are used for all children being transported.
Regulation Description
340:110-3-305(g)(3) - Children remain properly secured in child passenger restraint system, such as a car or booster seat or an individual seat belt complying with applicable state laws, unless the vehicle is exempt, per (5) of this subsection. Children do not share a seat belt.

Transportation - Maximum vehicle sitting space is being exceeded on the front row of the van.
Plan to Correct
A plan of correction was received 9/30/24 from the program's owner/director Felicia Logan - There is a 4th bench seat that will be reinstalled in the van for use. The vehicle seating capacity will not be exceeded.
