Licensing observed one teaching personnel with an OPDL that expired on 4/16/2026 to only have six verified training hours.
Licensing observed one teaching personnel's OPDL expired on 4/16/2026.
Licensing observed two swing set made for infants was not anchored into the ground.
Licensing observed in the Rose rock classroom (2yr's and older) 14 children and 1 personnel making the classroom out of ratio. Teacher stated the other teacher went to the kitchen to heat up lunch,

1.) *Serious* Supervision: Child left outside unattended.
Plan to Correct
In addition to the safety plan created on 8/23/24 we have added an additional staff not attached to a classroom while children are outside so they are able to take children to the bathroom, or make necessary inside runs. We have created a student lanyard system that goes in and out with the students so all students are accounted for while on the playground. The lanyard's have the students picture names, and classroom on them.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

I have reviewed licensing requirements for reporting to licensing worker on next business day I have specially paid attention to reporting the next business day. In the Future licensing requirements for reporting will be maintained.
