One personnel was caring for infants and had taken only 1-hour Safe Sleep Practices for Caregivers: Reduce the Risk of SUID unapproved and informal training.
The program is not maintaining supervision for two awake infants in cribs in the sleeping room by not having a personnel present in the sleeping room.
A four year old was put in the infant classroom as discipline for not listening while the class the child had been in went outside.
The program had no personnel in the back sleeping room with two awake infants who were in their cribs. The infants were moved in the front infant classroom, but that made the personnel in that classroom have five children with four infants and one four-year-old which the program continued to be over ratio by one child. Outside there were two classrooms, one of the classrooms had one personnel with seven one-year-olds.
One personnel did not have all infants in the front room of the baby area with them, and is not maintaining supervision by not being near enough to intervene with the two awake children in the back room of the baby area.
One personnel does not have a current OPDL and has not submitted her OPDL for renewal.
One personnel employed longer than 90 days has not completed ELCCT yet.
Two employed personnel do not have current CPR/FA certifications.
Program was not maintaining ratios in the back room of the baby area with two awake children in the back room and no personnel was present in the back room of the baby area.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. The children's health and safety is of utmost concern to our program. All protocols regarding these regulations have been reviewed by all staff and are monitored and maintained but he director and assistant director.

We are following the safety plan of always having at least two personnel on the playground at all times when children are present. Personnel are aware of how many children are in each classroom before heading out to the playground, that information is communicated to any other personnel who arrive for breaks or when taking over the group. When it is time to come inside all children line up by the wall and are accounted for before opening the door. Children are then counted again once inside. Personnel hold the hands of any children who tend to stary from the group when making transitions.
