One teacher employed by the program for more than 12 months has an expired PDL as of 1/10/26.
The program's registry profile is not being maintained as a teacher that was reported to no longer work for the program as of 8/1/24 is still showing on the program's profile with the same hire date of 3/20/24.
One teacher who has worked for the program for more than 3 months does not have documentation current first aid/CPR training.
Immunization records or exemptions were not available for four children whose records were viewed today.
The program does not have documentation of the current driver having completed this training.
Licensing observed a bowl of pineapple rings without a cover and a ham that was not covered properly in one of the refrigerators.
These forms were not on file for the four children whose files were viewed today.
Several cots in the 4 year old room do not have sheets present while children are resting.
There are several waffles present in a bowl in the refrigerator without a covering.
There is a slide and a climber present on the playground without impact absorbing material.
An electrical wire is hanging from the building in the outdoor play area with part of it laying directly on the grass. The ends of the wire and still attached to the building with a portion disconnected and hanging on the ground.
The swings removed from the swing set in the outdoor play area are accessible on the ground next to the swing set and could pose a strangulation hazard.

She states she will be taking further steps to ensure the reputation of the program remains intact. She states she will ensure staff always put the health and safety of children first and foremost.

Complaint:
Hazards - A staff person was actively using a vaping device in close proximity to children.
