The program did not have documentation of having completed the physical environment checklist within the past 12 months.
The program does not have documentation of having completed the equipment inventory within the last 12 months.
The director has not obtained the required number of professional development clock-hours to maintain a current ODC. Director has completed 8 hours of annual professional development.
Four teaching personnel, including director, have not obtained the required number of professional development clock-hours to maintain a current OPDL certificate
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. Complaint:
Hazards - A staff person was actively using a vaping device in close proximity to children.

She states she will be taking further steps to ensure the reputation of the program remains intact. She states she will ensure staff always put the health and safety of children first and foremost. She states she will be having a discussion with staff regarding vape pens and any other hazardous devices they could potentially be bringing into the program.
