Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Personnel job positions are not listed correctly on the Direct Care Organization, therefore the program is not maintaining all items listed on the Direct Care Facility Handbook.
At the time of Licensing's visit, the program did not have documentation of 1 staff not having obtained FA/CPR certifications after being employed for longer than 3 months.
Licensing observed the director not maintaining the direct care organization with updated personnel at the time of visit.
Licensing observed impact material to not be a depth of at least 6 inches by outside climbing swing set. Licensing observed that if a play structure is over 4ft high, then they will need to maintain impact material (the rubber mulch) of at least 6 inches. (page 82 of the requirements book).
