OIG audit completed 5/9/2025. Provider did not ensure that the in/out swipes were made correctly and failed to ensure that accurate time and attendance information was recorded on the POS machine or EBT app by the client.
Licensing observed changing pads in the 2 year old rooms both have tears in them.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. Complaint:
Personnel (SERIOUS) - Personnel did not demonstrate prudent and responsible behavior by having a bottle warmer cord accessible to a child who pulled the cord and hot water burned the child's arm.
Plan to Correct
On the intial complaint visit, staff put cable cord holders on the wall to provide a track on the wall that the cord is ran thru . Ensuring the cord can never be within reach of a child.

340:110-3-284(c)(2)(C) - demonstrate prudent and responsible behavior reasonably ensuring children's health and safety;
Complaint:
Hazards (SERIOUS) - A bottle warmer's cord, that was filled with hot water, was accessible to infants causing a burn to an child's arm when the cord was pulled and hot water spilled onto an child.
Plan to Correct
Bottle warmer was also moved to the center of the cabinet further ensuring its inability to be reached.
