Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
One member of staff, that has been on staff for more than three months, has expired First Aid/CPR certifications.
One teaching personnel that has been employed longer than 12 months, has an expired PDL certificate.
Two teaching personnel that have been employed with the program for more than 90-calendar days, have not completed ELCCT training.
One teaching personnel that has been employed by the program for more than 12 months has not completed enough training hours to maintain at least a level 1 PDL certificate.
Two wooden slats on west side of fence are broken with jagged edges present.
Two new multi-axis swings (one blue; one black) do not extend six feet plus the length of the chain on the sides; swing set frames and other frames are impeding the areas.
Large amount of new outdoor play equipment does not have impact-absorbing material in place, as follows - metal swing set with two single-axis swings; metal activity set one single-axis swing and one double axis swing (blue), monkey bars taller than four feet, and two pull-up bars taller than four feet; free-standing metal monkey bar apparatus; wooden activity set with two single-axis swings, rope climber taller than four feet - only grass and firm dirt are in these areas.
The program has no documentation for a current fire inspection; the last fire inspection report on file is dated 1/11/2021.
Director does not have current CPR/FA; expired 10/31/2023
Director has not obtained the required number of clock-hours to maintain a current Director's Credential; credential expired 11/16/23 and no training hours could be provided for the 12 months prior to this date.
