Plan to Correct

We were in the process of getting one of our two hot water tanks repaired at the time of the complaint. We were just waiting for a part to arrive. Part arrived and was installed on 2/9/24. However, to prevent any further issues, we moved forward with a complete hot water tank replacement. Installation was completed on 3/12/2024.

Plan to Correct

We will be reviewing with all staff members the proper name to face protocol when entering or exiting the building with students. We will be reviewing the importance of visual playground checks and counting your students multiple times throughout the day but especially during times of movement. Teachers will be reminded that they are required to code the front office when they are going outside and when they return so that we can monitor their movements and be aware of where classrooms are at all times. This will be a training opportunity for all staff within our facility to prevent further incidents in the future.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
