Plan to Correct

Directors (ONLY) will notify the parent (that enrolled the child) that someone (not on paperwork picking up). We will NOT release a child without picture ID (gov'nt) and email from parent that was contacted. Parent must come in next day to add to forms "for pickup". We will monthly call to check for phone numbers or weekly if contact numbers change. Frequently updated emergency + contact information required monthly and pick up. We terminated employee that violated our company policy and procedure and DHS "licensing" that results in this misfortune and violation. We also eliminated position. We extensive training for safe transition during everyday operation for all individuals picking up or visiting students/children to keep in files permanently.

340:110-3-280(d)(1)(C) - a child or individual at the pick-up location who contradicts previously established pick-up arrangements;

Plan to Correct

Staff member terminated. Position eliminated. Only Directors follow through with contacting parent that enrolled child, will document call, check and copy government issued Photo ID for file verify that they are on pickup list. If not they will not release. Child next day forms must be updated.

340:110-3-284(c)(2)(C) - demonstrate prudent and responsible behavior reasonably ensuring children's health and safety;

Plan to Correct

No child/student will enter or begin prior to all forms and information being completed, date, and signature. Director and designated staff must review and enter into system along with hard copy before child starts. Parent/child program handbook orientation and delivered to all families with signed acknowledgement of receipt of handbook.

As per licensing requirements we comply with all components. Substitutions are made when food is delivered expired. All menu's will be adjusted when substitutions are made and all changes will be compliant with CACFP (federal program). We project to be approved on CACFP program by 11/30/2023 per Kassandra Reddell.

Regulation Description

340:110-3-298(d) - Weekly menus. Menus of program provided meals and snacks are closely followed with reasonable substitutions permitted. Menus are posted, per OAC 340:110-3-281.1(c) and (e).

Plan to Correct

Highly qualified staff (assistant director) was terminated. Two or more staff will be counted in ratio in each class at all times. Staff training was held on ratio, supervision and transitions.

Regulation Description

340:110-3-291(c)(2) - Supervision. Teaching personnel supervise children constantly.
