Immediate Correction: Each child was moved to crib after picture. Teachers were reminded on Safe Sleep policies. Preventative Plan: Ongoing Safe sleep training for staff. Continued monthly staff meeting. Ongoing: Director and Assistant Director will perform daily safety check in classroom OKDHS coaching is used for extra training. Signs were hung in classroom attached in email.

Immediate Correction: All infants are now placed on their back during every nap, regardless of their rolling ability. Staff were reminded of this safe sleep policy. Preventative Plan: Staff will be reminded again of Safe Sleep policy at upcoming staff meeting. Director and or assistant director conducts daily checks. Ongoing monitoring: Daily sleep checks are conducted daily by director & assistant director.

340:110-3-296(b)(2)(C)(ii) - Infants who are able to turn themselves over, are placed initially on their back for sleeping, but may turn themselves over and sleep in a position they prefer.

Plan to Correct

Immediate Correction: Staff were reminded of time limit in equipment. Preventative Plan: Director purchased timers for the classroom. Each child will have a timer. It will be set for 19 minutes child will be moved at that time if not before. Ongoing monitoring: Director and assistant director will continue with daily safety checks in the classroom.

Regulation Description

340:110-3-289(j)(3) - do not remain for more than 20 minute increments in equipment restricting freedom of movement, such as rest equipment, swings, high chairs, or stationary activity centers, except while eating in high chairs or similar stationary equipment. Children only sleep in appropriate rest equipment, per OAC 340:110-3-296.

Immediate Correction: Director started clocking in/out on the Procare system that center uses. Preventative Plan: Director will print time sheets to have available. Ongoing monitoring: Director will continue to use Procare.
