Licensing observed a teacher, counted in ratios, leave a classroom to let a parent in the door, which put the classroom out of ratio.
One trash container observed in the toddler room with soiled training pants/diapers did not have a lid on it.
CCLS observed mouse droppings in the kitchen cabinets where food and dishes are stored.
Program Director has not been present for the past 6 monitoring visits that were complete by both OK Child Care Services and Cherokee Nation Licensing. The monitoring visits where Ms. Tracy has not been present include 8/1/23, 12/15/23, 4/16/24, 7/19/24, 11/5/24, and 11/19/24.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Per the Notice to Comply dated 10-04-23: The Program had been sending the Daily Reports for Infants & One Year-Olds via text message due to COVID. The Program will return to paper daily reports.
Regulation Description
340:110-3-281.4(b)(2) - Infant and 1-year-old daily record. Daily records for 1-year-olds and younger are available in the child's classroom and include prompt documentation of eating, diapering, and sleeping activities.
