Personnel job positions are not listed correctly on the Direct Care Organization, therefore the program is not maintaining all items listed on the Direct Care Facility Handbook.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
Complaint:
Personnel- A staff person left an unqualified person alone to supervise children.
Plan to Correct
"I will sure the staff is qualified to be left alone with the children".

340:110-3-284.2(a) - General. In addition to the position specific requirements in this Section, personnel meet the general requirements, per Oklahoma Administrative Code (OAC) 340:110-3-284.

Regulation Description
340:110-3-281.4(b)(7) - Medication permission. Signed parent permission for each medication is obtained, prior to administration, on an OKDHS or program form containing the same information.
