Two staff on site without verification of completed CPR/First Aid training.
Medication permission forms were not completely filled out by the parents and some forms were not signed by parents.
5 diaper creams in Infant B was not labeled with the child's full name.
Children were playing on the boards surrounding the mulch on the preschool playground. The boards are loose and broken.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Inappropriate Discipline (SERIOUS): A child was grabbed, forced to move, and slammed which resulted in the child bumping their head on the wall.
Plan to Correct
We retrained all teachers on how to help children with discipline issues. We immediately removed the teacher from the situation and terminated him from Kindercare and reported it to DHS on the day it happened.

Discipline and behavior guidance (SERIOUS): Staff threatened to use inappropriate discipline to children.
Plan to Correct
The teacher who used the threat of inappropriate discipline has been terminated. We will retrain all teachers on understanding and responding to challenging behaviors appropriately by Friday May 9th.
