For future compliance, will post a list of staff expiration dates so it's not missed again. Regulation Description CPR and first aid. Cardio-pulmonary resuscitation (CPR) and first aid certifications are age-appropriate for the children's ages accepted into care and are from approved sources listed on the OPDR website. Non Compliance Observed Medication permission forms were not completely filled out by the parents and some forms were not signed by parents. Labeling and storing
Non Compliance Observed 5 diaper creams in Infant B was not labeled with the child's full name. Non Compliance Observed Children were playing on the boards surrounding the mulch on the preschool playground. The boards are loose and broken.
