One staff member was hired on 9/29/2025 and has not completed CPR First Aid training.
Vehicle maintenance is expected to be conducted at least quarterly for program vehicles and personal vehicles used regularly for the program. The vehicles quarterly inspection should have been conducted at least between the months of October 2025 and February 2026. The vehicles' last quarterly inspection was conducted on 7/7/2025.
Two children did not have compliance file notification forms in their records. One child's compliance file notification was dated 1/8/2025.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Plan to Correct
We are having Environmental Pest control to come out and spray on August 14th, 2025. And we will be having them come out ever month to spray so that we will not be having any more problems with ants. I will also being doing daily checks around the building to see if I see any in between them coming out. If I do see any in between them coming out. If I do see any before schedule time for them to come out then I will call them back out to spray.

340:110-3-304(i) - The facility is free of pests, such as rodents and insects. Safe, effective measures minimize the presence and protect against the entry of pests.

Additional Non-Compliance Found During Investigation: 4.) Facility - One classroom has peeling paper and holes on the walls. Two other rooms in the program have holes in the walls.
Plan to Correct
I have contracted a contractor by the name of Handy Dan's Service to come in and take all the old walling down, fix what is behind the wall and put new walling up where the termites had been. He will also be taking the wall down and putting a new wall up where the paper was coming down. He said that it will be finished by August 15th, 2025. I will keep a look out for anymore places that might need to be fixed. If I do I will call him back immediately.

Additional Non-Compliance Found During Investigation: 4.) Records - Children's attendance in the infant/toddler room is not documented with arrival and departure times.
Plan to Correct
We at Creative Children's Learning Center will be implementing a children's attendance sheet. The daily attendance sheet will be posted every week so that the parent/teacher can document the time the child/children arrive and/or depart to and from the daycare with the actual time that they arrive/leave every day. The attendance sheet will have the child's full name, arrival/departure time, and a personnel signature or initial of the parent and/or teacher signing in/out for the day. We will maintain these documents for at least 12 months.
