Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
*SERIOUS* 11/19/24 Self Report: Supervision - Personnel left a child alone, without any other Head Start personnel, sitting on the brick wall in front of the elementary were all of the buses were parked. The child rides the bus to go home. The personnel left the child (bus rider) to take another child who is a car rider to child's parent to pick up and sign child (car rider) out. The child's (car rider) parent was parked in the Head Start parking lot approximately 103 feet away from the child (bus rider) left alone in front of the elementary. When personnel returned to the child (bus rider) that was left alone, sitting on the brick wall, the child (bus rider) was no longer there. Child (bus rider) was missing for more than 5 minutes before located.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
