Licensing viewed multiple children resting on mats without a sheet covering the surface.
The program does not have documentation of having completed an equipment inventory in the last 12 months.
The program does not have documentation of having completed the physical environment checklist in the last 12 months.
Immunization records or an exemption was not on file for one child in care today.
Upon Licensing arriving at 7:00 am all children were in one classroom for which ratios were not met – 17 children, ages 1 and older with 2 staff, 4 children @ age 1. At 7:43 AM, the 3s and older classroom had 21 children with only one staff, four children @ age 3. At 8:53 AM, the infant room had six infants present with only one staff.
A staff working with children and employed since 2-25-25 does not have a completed background check or a background check status of in process.
Last documented smoke detectors and carbon monoxide check was documented on the monitoring summary 10-30-24.
Fire, Tornado drills were last documented on 10-30-25 monitoring visit.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. Staff-Child Ratios - The program is seriously understaffed during early morning hours.
Plan to Correct
At time of visit school was understaffed due to staff calling out for the day and not following call in policy of two hours prior to shift speaking with a member of management to verify shift could be covered before not coming in for the day. Meeting was held with all employees to discuss this policy and importance of following so this does not occur again. School will update schedules as needed based on needs of school to insure moving forward staff ratios are meet at all times.
