340:110-3-280(d)(3)(A) - daily happenings, including the infant and 1-year-old daily records as maintained, per OAC 340:110-3-281.4(b);

At time of visit school was understaffed due to staff calling out for the day and not following call in policy of two hours prior to shift speaking with a member of management to verify shift could be covered before not coming in for the day. Meeting was held with all employees to discuss this policy and importance of following so this does not occur again. School will update schedules as needed based on needs of school to insure moving forward staff ratios are meet at all times.

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Notice to comply received from owner/agent, Kelsey Taft - Staff/child ratios are always maintained in all classrooms at all times. Management will make sure ratios are met at all times throughout the day to meet the needs of the building

Notice to comply received from programs owner Kelsey Taft on 6/28/24 - Personnel mentioned in complaint is no longer employed with center. All current and future personnel are trained on appropriate positions during bottle feeding.
