One staff CPR First Aid certification expired 7/31/2025. Five staff with hire dates of 11/18/2024, 5/21/2025, 6/30/2025, 1/24/2025, and 6/12/2025 did not have CPR First Aid certifications.
Four staff with hire dates of 6/30/2025, 7/21/2025, 1/24/2025, and 6/12/2025 did not have ELCCT training.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Rest time- Infant in an inappropriate sleep environment
Plan to Correct
To reinforce safe sleep practices, swings have been removed from the infant room. Individual staff meetings have already covered safe sleep guidelines. We will be reviewing these guidelines again at the staff meeting on August 13th. All staff will also complete a safe sleep refresher course beginning with those who work in the infant room and all others throughout the year. This will be monitored with cameras and classroom observations.

340:110-3-296(b)(2)(B)(ii) - When an infant falls asleep in equipment other than appropriate rest equipment, the infant is immediately moved to appropriate rest equipment.

Additional Non-Compliance Found During Investigation: Records: Diaper changing was not documented consistently for one infant.
Plan to Correct
All staff will be re-trained on the importance of prompt and consistent documentation of diaper changes, feedings, and sleeping times. This will be done verbally to all infant and toddler teachers immediately and to entire staff at next staff meeting on 11/7/23.

340:110-3-281.4(b)(2) - Infant and 1-year-old daily record. Daily records for 1-year-olds and younger are available in the child's classroom and include prompt documentation of eating, diapering, and sleeping activities.
