One member of staff employed for more than 12 months as an assistant caregiver has an expired PDL certificate.
One member of staff employed for more than 12 months as an assistant caregiver has not completed 12 clock hours of professional development within the past 12 months.
One member of staff who shows as having been employed at the program since 3/24/25 has been working in the home with an "incomplete closed" background check.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Plan to Correct
Owner/director willensure to check that 2 or more clock hours have been obtained in safe sleep training prior to the caregivers start date. Owner/director will ensure that the trainings are state approved by checking the OPDR website.

340:110-3-85(b)(1) - General. The caregiver is a responsible, mature, healthy adult capable of understanding and complying with licensing requirements, and meeting children's needs. The caregiver demonstrates prudent and responsible behavior reasonably ensuring children's health, safety, and well-being. Caregivers interact with children without physical, psychological, or emotional punishment, mistreatment, neglect, or abuse.
