Licensing observed that one child was missing immunization records.
Licensing observed that fire extinguisher expired in November 2024.

Caregiver Requirements: Background Investigation is not completed on adult who resides in home.
Plan to Correct
A Notice to Comply completed during visit. Notice to Comply with the following plan of correction: Caregiver scheduled fingerprint appointment during visit. Caregiver stated that adult will move out of the home and not be allowed to live in the home until background investigation is completed. During visit, the adult left the home. In the future, Provider stated she will follow requirements on background investigations in order to maintain compliance with requirements.

340:110-3-85(c)(1)(E) - adults living in the facility, prior to authorization to operate or moving into the facility of an existing program. This includes children, who become 18 years of age while living in the facility; and

Additional Non-Compliance Found During Investigation: Caregiver Requirements: Caregiver did not report by five calendar days of an individual who moved into the home.

A Notice to Comply completed during visit. Notice to Comply with the following plan of correction: Provider stated she will notify Licensing of individuals who move into the home with in 5 days. In the future, Provider will report any individual who moved into the home as required to maintain compliance with requirements
