Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider states that she has an upcoming appointment August 8th for them to get shots and records updated. Provider states that in the future she will maintain this. General requirements
Non Compliance Observed Licensing observed that one child's child information sheet was missing. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Primary caregiver states that she will either find it or have parents fill out new one. She will send copy to child care licensing worker. Provider states that in the future she will make sure all records are kept where licensing can observed them. Regulation Description General requirements. Children's records are maintained on Oklahoma Human Services (OKDHS) provided forms or on forms containing the same information. Records are current and easily accessible. OKDHS staff are allowed access to all records. Plan to Correct Date 2024-07-15
