The mattresses in all four cribs are not firm.
Swing set is not anchored. The slide is unsteady & the metal supports for the swing set can be moved easily.
No lesson plan posted in classroom.
Observed hole large enough for a small child to fit thru. Some sections of the repaired fence are not secure.
Gate can be easily pushed open, allowing children to get out of play area.
Use zone for swings do not extend at least six feet.
No impact material under the two swings.

The outside fence has barbed wire along the top.
Program fence has a hole large enough that a small child could fit through.
