Plan to Correct

As an immediate corrective measure, the bench on which the child was seated has been removed effectively from the outdoor play area immediately. Soft protective cushions/mats will be placed beneath and around any infant or child who cannot yet independently sit, stand, or walk with stable balance, in all indoor and outdoor areas, and will be checked daily. A minimum of two teachers will be present at all times, indoors and outdoors, for any group that includes a non-mobile or newly mobile child, so that supervision of a seated or standing child is never left uncovered when a teacher must step away to assist another child; the facility will maintain the correct staff-to-child ratio at all times with no exceptions. All staff will be retrained on active, close-proximity supervision and a formal attention hand-off protocol between teachers before either one's attention is redirected. The Site Director will conduct regular, unannounced spot-checks of supervision and ratio compliance for the first 60 days, with a follow-up review at 90 days to confirm these measures remain in place and no similar incident occurs.
