Vehicle being used for transportation does not have program name on the vehicle.
Transportation attendance has not been maintained since transporting children to school. .
Transportation itinerary has not been completed of approximate vehicle location while transporting to children's schools.
Emergency contacts and child information of each transported child not on paper and in vehicle.
School age classroom did not have current lesson plan posted.
When licensing worker entered infant classroom infant teacher was on her personal cell phone. She immediately put cell phone away when she saw licensing worker.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Owner had discussion with staff regarding plan if van driver/ office manager is not at the program to transport. Van used for transportation will be left at the program and be available for staff to use to transport children to and from school.

Reporting- Program did not notify parent when child was not taken to school when made prior arrangements with the program.
Plan to Correct
Owner had discussion with staff regarding plan if children were not taken to school. Staff will contact parents of the children that were to be taken to school promptly if program is unable to transport children to school.
