The use zone for the tall metal dome climber is not six feet from the perimeter in all directions; this is situated close to the back fence.
A tall metal dome climber (higher than 4') does not have required impact material in place; only grass is present.
A staff supervising children alone on the playground was sitting in a chair and participating in electronic device use. The children ages 3 and 4 where hitting the sides of the building and the gutters with large sticks. The staff was sitting on the concrete area on the north side of the playground and the children were by the side entrance to the building on the west side. The side entrance is blocked by the building and can not be seen/supervised from the concrete area.
One master teacher has a PDL which expired on 3/22/25.
The programs central detection system was last inspected and tagged on 2/5/24.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Supervision - A child was left alone in a van in the parking lot upon returning to the center after school; staff were unaware of the child's whereabouts until parent arrived to pick up the child; the child was found sleeping on the van and had been there since 4 PM.
Plan to Correct
Notice to comply completed at the program on 3/5/25 - Staff transporting children at the time of the incident was terminated from employment. Transportation requirements will be reviewed with all staff transporting children and requirements will be followed at all times. An additional staff will be assigned to check the vehicle upon returning to the program long with the driver checking the vehicle. Children's transportation attendance records will also be reviewed by two staff to ensure all children are accounted for.

340:110-3-287(h)(1) - Children are never left unattended in vehicles.
Complaint:
Additional Non-Compliance Found During Investigation: Personnel (Responsibilities) - Staff did not ensure transportation attendance records were accurate for children exiting the vehicle.

Regulation Description
340:110-3-281.2(d)(6) - Child passenger check. Before leaving the vehicle and in addition to the transportation attendance, the driver or last personnel in the vehicle documents completion of at least two methods of inspecting all areas of the vehicle for any remaining children to ensure no children are left on the vehicle.
