  • At the time of licensing's monitoring visit the last fire inspection was dated 10/17/23.
  • At the time of licensing's visit, the program did not have documentation of 1 staff having obtained FA/CPR certifications after being employed for longer than 3 months.
  • During the walkthrough licensing observed the non-porous diapering surface to have tears and rips in the surface.
  • During licensing's walkthrough, the playground did not have impact materials of at least 6 inches in depth underneath the infant swing and climber that is higher than 4 feet.
