The program does not have documentation of having completed the equipment inventory in the past 12 months.
The program does not have documentation of having completed the physical environment checklist in the past 12 months.
The programs director has a director's credential which expired on 12/6/24.
The use zones for the metal dome climber do not extend six feet from the perimeter in all directions; concrete is present in part of the use zone space.
The programs first aid kit does not contain a thermometer.

Complaint:
Supervision - During a field trip to a large park in downtown Oklahoma City, staff failed to maintain visual supervision of all children in care. Two children, both age 4, became separated from the group and were temporarily unsupervised. Park security located the children and returned them safely to staff.
Plan to Correct
Notice to comply completed 10/28/25 - Staff will review safety procedures for field trips and supervision in general. The program will be temporarily suspending field trips. When field trips resume all 4 employed staff will attend to ensure supervision. The program will have all children wear white shirts with Angelfish written on the shirts. Children will also wear name tags on the back of their shirts. The program will only attend field trips to locations that are less busy then the large park and only locations indoors. The program will ensure all staff follow supervision requirements at all times.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
