- Licensing observed three staff employed over 12 months to not have a current PDL.
- Licensing observed three staff no longer employed at the program, to be listed on OPDR.
- Staff employed on 2-1-23 has 9 hours of training listed on the Registry.
- Licensing observed one of the gates to not be secure. The bottom section was not connected to the fence creating a gap between the other section of the gate. Licensing slightly pushed on the gate and it opened.
- Program could not provide current copies of the Physical Environment Checklist and the Equipment Inventory checklist during the visit.
