One child present in the infants and older mixed age classroom was missing an immunization exemption form.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Supervision - Supervision was not being maintained at all times.
Plan to Correct
All staff rewatched the office of head start active supervision training. https://headstart.gov/video/introduction-active-supervision.
All staff completed the association training kit, including the self reflection tool and active supervision implementation plan. These components were completed collaboratively at the classroom level, with each classroom team developing a shared implementation plan.
All staff reviewed and resigned the active supervision policy, as well as daily monitoring documents, face to name procedures, and the cell phone policy.
Teaching staff received retraining on all sections outlined in the Sunbeam training checklist. As we were not clear on the complaint topic, we immediately re-trained on all areas once notified of the complaint. A sample checklist is attached.
The door handle system was replaced.
Posted reminders throughout the center were refreshed. A sample attached. Facilitated jeopardy active supervision training for all staff. Sample attached.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
