Plan to Correct

We have implemented hourly name to face on Bright Wheel & the personnel in charge checks the classrooms as a backup to ensure we are in ratio at all times.

Regulation Description

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

We have stopped taking the toddlers to the bathroom that has stall doors. They will now be using toddler potty training potties that are clean and sanitized after each use.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

We terminated both staff who were in the room during the inappropriate discipline incident. We also held a staff meeting on 6/26/2024 to train all staff on what appropriate discipline is and that our number one way to do this is redirection.

Regulation Description

340:110-3-288(c)(1)(F) - restraining or tying to restrict a child's movement by any means other than holding. Holding is used only as long as necessary for the child to regain self-control.
