Strategic steps were implemented, which included questioning the teachers involved, reviewing video footage, notifying families, and documenting disciplinary actions related to the incident. A safety audit was conducted on May 20th at 8:00 am and action steps were identified to create additional barriers to parking lot and road. We program will ensure that there is always a staff member who has line of sight of the gate while the children are on the playground. We will have a training on supervision for all staff once we start back up in August and we will ensure all staff are conducting head counts and name-to-face before they go outside, while outside on the playground and once they come in.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

We will report to Licensing within 24 hours if an incident were to occur exposing children to imminent risk of harm, such as a child leaving the facility without personnel's knowledge.

Plan to Correct

We are firmly implementing our Procare app. This gives the teachers access to communicate directly and frequently with the child's daily routine, diapers, meals, activities and any kind of report. This also gives the parents access to the teacher to communicate directly through a message system and the teacher can reply back to the parents as those messages come. All of the meals have been changed to adjust and cater to all child's needs and accommodate the correct portion sizes. The teachers are given the opportunity to have additional training to know the correct sizes and what is appropriate for the age of the child. We are also sitting and staying with the children while they eat to make sure we are all eating all the foods and staying safe the whole time. If an accident is to occur the teachers are to ask for management. The manager will bring an incident report and the teacher can fill it out then giving it back to the manager. Pending the accident the teacher will either send a message via procare informing the parent of the incident and letting them know to stop by the office at pick up to talk to the manager and sign the report. If the incident is immediate the manager will call the parent and notify them direct in the moment. Then it is documented and the parent with either choose to come immediately or wait til normal pick up. The parent will still stop by the office to speak to management and sign report. These changes are to make meals more healthier and safer. The implementation of the Procare app is to assure that the children are present and a trail of their daily routine. The incident reports is to document all incidents and to make sure that all parties, families and children and staff are aware of said accidents and are documented for all involved. These actions are to take effect immediately.
