Cabinet in bathroom in 3 year old classroom has a broken latch. Bleach all purpose cleaner and other counter cleaners are underneath this unlocked cabinet.
No thermometer in fridge in two year old classroom. No thermometer in fridge and freezer in three year old classroom.
One diaper changing pad in the two year old classroom has tears on it.
Hazards were found under the sink in two bathrooms. Lock on the cabinets did not catch and lock the door. The cleaning supplies in two cabinets are accessible to children.

Complaint:
Supervision - Serious - Child left on playground unattended.
Plan to Correct
A "playground sweep" has been implemented. The teachers were all trained to complete a sweep of the playground as their students are gathering to go inside. The teachers were instructed to look in common places where a child might hide or be hidden from view. The teachers were instructed to count the children twice and then again as they enter the building, and one more time as they arrive in the classroom. The teacher will have a daily class checklist to keep track of the students that they have. If more than one class is on the playground at a time, the classes will go in separately, to minimize the chances of students getting mixed up with where they should be going.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
Complaint:
Diapering & Toileting - Diapers not changed appropriately when children are wet or soiled.
Plan to Correct
Teachers were reminded of scheduled diaper changing times and reminded to keep a close eye on the posted schedule. The teachers were also retrained in checking and changing diapers throughout the day, and not just at scheduled times. When a diaper check or change is conducted, it is documented on Brightwheel for the parents to see.

Additional Non-Compliance Found During Investigation: Records - Incident report was not completed for known injury to child in care.
Plan to Correct
The director will perform weekly checks in each classroom, checking the student information sheets in the classroom binders and asking the teachers about the students they have in the room. These checks will be done during all shifts, to be sure each teacher is spoken with. Their ages and any allergies, as well as, if they are taking any medications are some topics, but not all, that will be asked about. The director will also spend a couple minutes during this time just discussing the children with the teacher so they can both be aware of anything that may be going on with them. The classrooms will have 10-15 minutes with both teachers present when it's time for a shift change. The two teachers will spend this time exchanging information about the children and their daily experiences in the room.

340:110-3-281.4(b)(9) - Injury or poison exposure report. An OKDHS or program form containing similar information is completed for each injury or poison exposure occurring while in care.

Plan to Correct
The director, or person in charge, will now be the person to fill out all incident reports, along with the personnel present during the incident. These reports are signed by all personnel completing them, as well as the parent. The parent is notified of the incident immediately on Brightwheel and then asked to sign the report upon pick up. Incidents resulting in more serious injuries will be reported to the parents immediately with a phone call instead of using Brightwheel.
